Tri-Heart Plus, for the prevention of heartworm disease and treatment of roundworm and hookworm infections in dogs. Tri-Heart Plus chewable tablets can be easily administered once a month. 6 doses. Up to 25 lbs, 26-50 lbs, 51-100 lbs. Active ingredient ivermectin/pyrantel.
The SmartPak Heartworm Preventative Guarantee
If your dog tests positive for heartworms while being administered a heartworm preventative purchased through SmartPak, we will refund the purchase price or provide replacement product and reimburse reasonable diagnostic and treatment costs up to $1500 to return the dog to negative heartworm status provided the terms of this guarantee are met:
- You must have purchased the heartworm preventative from SmartPak
- You must have administered the heartworm preventative on a monthly basis according to the label and your veterinarian’s directions
- You must have your dog tested annually for heartworms
- You must have administered the last dose of heartworm preventative within 31 days of diagnosis
- You must have used the product before its expiration date as found on the package
When requesting reimbursement, you must provide all of the following:
- Proof of negative heartworm status within the last 12 months
- Proof of current positive heartworm status
- Itemized invoices and laboratory records for all diagnostic workup prior to heartworm treatment
- Itemized invoices and medical records for heartworm treatment
- Lot number of product used
In the unlikely event your dog does not accept one of our heartworm preventatives, SmartPak will either send a new package of the product or refund the purchase price.

Precautions
Store at controlled room temperature of 59 degrees to 86 degrees Farenheit. Protect product from light. Keep this and all drugs out of the reach of children. Caution: Federal law restricts this drug to use by or on the order of a licensed veterinarian.

In accordance with state and federal regulations, we are unable to accept returns on any pharmaceutical items.
For all expedited deliveries UPS is responsible for delivering the package on time. Certain circumstances can arise where UPS is unable to deliver expedites on time due to weather, remoteness of location, etc. All efforts will be made by SmartPak to ensure that packages arrive on time as ordered, but we cannot guarantee that UPS will always act accordingly. In particular, orders indicated for Saturday delivery cannot be guaranteed.
We strongly recommend that if the order is for a particular health emergency, customers should seek immediate assistance from their vet. If a UPS expedited delivery fails to arrive on time as ordered, SmartPak will refund the additional shipping charges incurred.
